Community Health Nurses

Community health nurses offer a range of services across WACHS.

Community health school nurses are skilled in assessing adolescent health needs and engage with young people to identify issues, risks and strengths. Community health nurses also provide advice, health education, brief intervention, immunisation, assist with access and referral to other services and provide follow up care as needed.

Young people can drop-in to a health centre or make appointments to discuss health and wellbeing issues.

Services are free and confidential.

At times, information may need to be shared to provide support and care and ensure the safety and wellbeing of a young person.

The school health service team encourages and supports young people to talk to their parents or guardians about significant health issues.

If you have a concern about your health or wellbeing, contact the community health nurse based at your school. When a community health nurse is not available in person, there are a range of online resources that you can access.
